Step 1: Check that bub is up to date with their health check and vaccinations
Step 2: Fill in a birth certificate package application form available from the clinic
Step 3: Return it to the clinic or to the address detailed on the form
Step 4: Births, Deaths and Marriages will arrange to post the certificate package to you at the address you nominate.
